  • Oliguria/anuria after starting a new nephrotoxic agent
  • AKI + eosinophilia + rash + fever - acute interstitial nephritis
  • AKI in the context of combined ACE-I/ARB + diuretic + NSAID ("triple whammy")
  • Rising creatinine after IV contrast in a patient with pre-existing CKD
  • Crystalluria/flank pain (acyclovir, methotrexate, sulfonamides)
