Red flags
- Fatigue + weight loss + night sweats - malignancy/lymphoma/TB
- Fatigue + pallor + bleeding - haematological malignancy/bone marrow failure
- Fatigue + focal neurology - consider demyelination, stroke
- Fatigue + chest pain/dyspnoea - cardiac cause (may be anginal equivalent, esp. in women/diabetics)
- Profound fatigue with postural symptoms - Addisonian crisis, severe anaemia
